Steve Checkoway 62b49f2d7f Fix compile and link errors. The relevant parts of the standard have been cited for those following along at home.
typename must be used for dependent name resolution as described in 14.6 [temp.res].
An elaborated-type-specifier with the class-key present is required ("shall be used") in a friend declaration for a class. 11.4.2 [class.friend] and 7.1.5.3.1 [decl.type.elab].
However, an elaborated-type-specifier cannot be  a typedef-name. 3.4.4.2 [basic.lookup.elab].
Lastly, the is no need for registration since "a definition for a static data member may be provided in a namespace scope enclosing the definition of the static member's class template." 14.5.1.3.1 [temp.static].

Steve Checkoway a5b9861532 Encapsulate extra stage logic. The condition of HasEarnedExtraStage() remains true (if it was earned) after the call to FinishStage() until the next BeginStage().
I still don't completely like this because after FinishStage(), if an extra stage has been earned, it is immediately in that stage which is sort of strange for SEval. Even worse, if an extra stage was earned in the past but not for the current stage, then the stage doesn't change. (E.g., earn extra stage but not second extra stage, or earn second extra stage.) So once an extra stage has been earned, SEval will be called twice for the same stage. A per-player  m_iExtraStagesPlayed could be added to fix this, but it doesn't seem worth it.

Steve Checkoway 101abde1d4 Specify exactly what is allowed in the strings according to what is valid lua. Double quoted strings are not supported. Require that function parameters have types. Steve Checkoway 725f2e7c27 Any given step can continue the combo, maintain the combo, break the combo, or outright miss. Those that break the combo aren't misses and shouldn't increase the miss counter but they should break the combo.
Fixes bug 1738253.

Steve Checkoway bfefade4f3 -Move player input together on the second page (this will probably be changed to an option for the metrics).
-Play (Gain|Lose)Focus on the icon only when the first player moves onto it/last player moves off of it. This doesn't work correctly when switching pages.
-Move the cursor at the beginning of switching page so that it can begin to animate along with the rest of the elements. This requires knowing where it is going so use the destination x and y of the corresponding icons.
