Glenn Maynard ae2c4f779c make toasty rare (having it come up once every few days is amusing;
having it come up twice in one song is annoying)

(easter eggs should be rare anyway, or they become distracting ...)

Glenn Maynard b00c8d5413 Remove HighComboAlert_OnceOnly. (Added this because I wasn't
sure which was right, and it looked like =true was the behavior it
was trying for, but =false is normal arcade behavior and I can't think
of any real reason for it to ever be true.)
