[GameCommand] Remove pushscreen

[ScreenOptionsMasterPrefs] Reset Judge and Life values to the ones in SM4
[ScreenNetEvaluation] add Score, Grade, PlayerOptions params to UpdateNetEvalStats message
[PlayerState] add GetHealthState Lua binding
small cleanup in statsman
