Allow libmad to be compiled in Windows.
...
This removes the need for an externally built library, thus officially removing the deprecated Xbox projects.
Other minimal changes were made to ensure consistency. Comments will be made in the PR.
Thanks to @Prcuvu for the initial push.

This is meant to be a safer alternative since
NULL can often be 0. Let's not rely on that.
And yes, I know this is a lot of files. This is
a safer thing to do in big commits vs for loops.
